National Futsal Championships – New forms released

The 2009 Futsal Nationals Website has been updated today to include various new forms for the event.

Official Nomination Forms – DOC (Due November 30 2008)
Player Registration Forms - DOC (Due January 2 2009)
Player Code of Conduct - PDF (Due January 2 2009)
